Most people who give up on HRV do so because the number jumps around and they cannot tell whether that means something or nothing. A good part of that scatter is real physiology you should not try to erase. The rest is measurement noise, and a surprising amount of it is in your control.
This guide separates the two. Fix the controllable half and you get a signal you can actually read; accept the rest and you stop chasing it.
Apple Watch measures heart rate optically: it shines light into the skin and reads what comes back, then derives beat-to-beat timing from that. Anything that lets the sensor lose contact — a gap, a slide, a shadow of air under the case — degrades the timing precision before any calculation happens.
Three things matter, in order:
Wrist choice matters less than consistency. Pick one wrist and stay there, because switching mid-week introduces a step change in your data that looks exactly like a physiological one.
Your watch does not record HRV continuously. It samples opportunistically, on watchOS's own schedule, and there are days it simply gathers very little. Two settings widen that window considerably:
It is worth being clear about the ceiling here: even with everything on, a consumer watch may record HRV only a handful of times a day. No app can score readings that were never taken. If your score seems to update rarely, that is usually watchOS's sampling cadence rather than a fault in the app reading it.
The strongest argument for sleeping in the watch is not that night-time HRV is more "correct". It is that night-time conditions are more repeatable. You are lying down, you are not moving much, you have not just had coffee, you are not mid-conversation, and the timing is roughly the same every night. Comparing Tuesday night to Wednesday night is a much fairer comparison than Tuesday's 3 p.m. to Wednesday's 9 a.m.
Battery is the usual objection. Charging while you shower and get ready, or for half an hour before bed, generally covers a full night without losing daytime wear. If you charge overnight instead, you will still get readings — they will just be daytime ones, with more scatter in them, and you will need a longer run of days before the trend settles.
Sometimes you want a deliberate measurement — a Breathe session, or a moment where you know the watch is sampling. If you do that, hold the conditions steady, because HRV responds strongly to all of them:
A large share of "inconsistent" readings are perfectly consistent responses to something real. Alcohol is the most reliable suppressor most people can see in their own data, often the night of and the night after. A short or badly broken night, a hard or unfamiliar training session, the first night in a new time zone, a fever coming on, a room that is too hot — all of these move HRV without any of them being a sensor problem.
You do not need a spreadsheet. Just knowing that Thursday was a late one means Friday's dip is explained, and you stop treating it as noise to be engineered away or as a signal to be alarmed about.

The last step is a habit rather than a setting. Consumer HRV samples are short, taken through skin, from a moving wrist, on an opportunistic schedule. Any individual value carries modest information. A week of values carries considerably more.
So the useful question is never "why was today 43?" — it is "is this week sitting below where I have been sitting?" That framing also makes the noise stop mattering. Scatter that would wreck a single-reading interpretation largely averages out across a run of days, which is exactly why a baseline is worth building.
